Form 3A is used when seeking approval for credit of an educational activity. Approval must be sought before beginning an educational activity.
Form 3B is used to seek approval prior to beginning an educational activity presented by a Non-Approved Provider. See Approved PDU/CEU Provider List above.
Form 1 is used for Educators interested in serving on the LPDC Committee should complete this form
Form 6 may be used by a Lake County ESC employee to appeal a decision made by the LPDC.
Form 7 may be used by any educator in Lake County to appeal a denial from their local LPDC.
Used by individual employees to keep track of their professional development activities.
Form 2 should be submitted for approval immediately following the renewal of your 5-year license and prior to completion of any coursework or activities. The Standards for Ohio Educators should be used in the completion of this plan (above).
Form 4 should be completed and attached to your proof of completion or attendance at a conference or workshop.
Form 5A is to be completed and submitted upon completion of an educational activity. Please attach your pre-approved Form 3A and Form 5B when submitting.
Form 5B should be used by the individual employee to record contact hours for one activity. Must be attached to Form 5A and submitted upon completion of an educational activity.
Form 8 may be used to transfer IPDPs and coursework to or from the Lake County ESC.
